For coaches & clinicians
We work with your team, not instead of it.
For physicians, physical therapists, athletic trainers, strength coaches and sport coaches. Refer an athlete or patient for hands-on assessment and treatment, and get back a clear account of what was found, what was treated and what comes next.
When to refer
Where a hands-on assessment adds to your work.
Cleared, but not moving right
The athlete has met return-to-play criteria but still compensates around the current or old injury.
The problem that keeps coming back
Recurring pain or tightness that improves with rehab, then returns under load.
Progress has stalled
Rehab or training has plateaued, and you suspect a joint or soft-tissue restriction is limiting it.
After surgery
In early post-operative rehab, in direct communication with the surgeon and within their protocol.
Before load increases
A developing athlete starting strength work, or an athlete entering a heavier training block.
In season
Maintenance care timed around competition, coordinated with the athletic trainer.
When the history or findings suggest something outside our scope, we refer to a physician and tell you.
What you get back
A conversation first. Written notes when you need them.
With the patient's permission, you hear from Dr. JC directly, usually by phone or message: what he found, what he treated, and how the plan fits with yours. A conversation often says more than a report. Written notes, in the format physicians and therapists expect, are available on request.
When
What you receive
After the first visit
A call or message covering the findings, the working impression and the initial plan.
As care progresses
An update when the plan changes, the patient progresses, or something needs your input.
On request
Written notes or a summary for your records.
Anytime
A conversation to talk the case through.
How to refer
Three ways, all simple.
01
The patient books directly
They book an initial visit online and note your name in the intake. Dr. JC contacts you after the first visit, with their permission.
02
You contact Dr. JC
Call or text (949) 393-9490 to talk through the case first. Or email jc@sportslabfl.us.
03
Send records ahead
If imaging, test results or your notes are relevant, send them before the first visit by email to jc@sportslabfl.us or by fax to (949) 691-0950.
No referral form is required. Patients don't need a referral to be seen; a referral simply makes coordination easier.
Scope and experience
Clear about what we do, and what we don't.
From 2017 to 2026, Dr. JC was the lead physical medicine provider for both Providence St. Joseph Health and Hoag Medical Group, working day to day alongside primary care, physical therapy, acupuncture, behavioral health and health coaching.
What we do
Hands-on assessment and treatment of joints, soft tissue and the sensory input the nervous system depends on; corrective and proprioceptive exercise; coordination with rehab and performance.
What we don't do
Replace the physician, surgeon, physical therapist or coach already in charge of the case.
Knowing the limits
When something isn't ours to treat, we say so and refer.
Join the network
Built one relationship at a time.
We're adding preferred partners in nutrition, sport-specific coaching (hitting, pitching, skating, golf and speed), and strength and performance, in Tampa and remotely. Partners are independent: they set their own fees, keep their own clients and are booked directly.
What we look for
- Work we've seen and trust, built on the same order: table, floor, field.
- Clear communication, with shared notes and handoffs when the patient allows it.
- Respect for every stage, including the ones that aren't yours.
No referral fees and no compensation, in either direction. A referral is made only because it fits the patient.
Questions
Will you take over my patient's care?
No. Dr. JC treats what the assessment shows and coordinates with you; the patient stays with the people already responsible for their rehab, training or medical care.
Can my patient keep seeing me while they see you?
Yes. That's how it works best. Care is planned around what you're already doing, so the two don't compete.
Do you need imaging or records before the first visit?
Not always. When imaging, test results or your notes bear on the problem, they help. Dr. JC will ask for them if they're relevant.
Is there a fee for referring or for being referred to?
No. We pay no referral fees and receive none, in either direction.
